Blue unit was one of the oldest units of this institute. From then it plays vital role in the field of orthopedic surgery and trauma care in Bangladesh. Renowned orthopedic surgeons of our country run this unit since the inception like Health Minister Prof. A. F. M. Ruhul Haque, Prof. Salek Talukder, Prof. M. A. Hannan, Prof. A. K. M. Eshaque and Prof. Kh. Abu Awal Rizvi. In 2007 it was divided in Blue I & Blue II. Prof. M. M. Bari took the charge of Blue unit II. After him the then Director Prof. Kh. Abu Awal Rizvi, Prof. Hasan Masud, Prof. Moinuddin Ahmed Choudhury run this unit successfully and make this unit famous in orthopedic arena of Bangladesh. Since October 2023 it was run by renowned Spine Surgeon of Bangladesh Prof. Md. Rezaul karim till now.

This is a versatile unit where all varieties of orthopedic surgery like Arthroplasty, Ilizarov, and Musculoskeletal oncology other than trauma care performed.
